Benefits and Overview

Montefiore Diamond Care is a managed long-term care plan designed for individuals who are frail but able to live independently at home with services and support provided by Montefiore Diamond Care. Join Montefiore Diamond Care and you will have access to coordinated health, social, nutritional and rehabilitation services as well as our team of nurses, social workers, service coordinators and other healthcare professionals in your community.

Care Management

When you join Montefiore Diamond Care, you will have your own care management team who will speak with you and your family to manage your day-to-day needs. The care team’s goal is to provide you with well-coordinated care. They can identify problems early and change your service plan to help you stay in your own home for as long as possible. Your care team will work with you and your family to provide you with the care you need.

Whenever you need assistance, your care team is there for you. You will receive regular health assessments and phone calls from a member of your care team. To help you stay active, you may receive rehabilitation therapy when medically necessary.

If you need nursing home care, it may be provided by a quality nursing facility in our provider network. Your care team will also help coordinate any hospital care or short-term rehabilitation. Montefiore Diamond Care will ensure that you get all the services you need through our comprehensive long-term care benefit package of covered services. Your care team can also help coordinate your medical services.
